DESCRIPTION OF REQUESTED SERVICES: Individual is seeking HPC provider for community access as well as ADLs (cleaning living space, cooking), and assistance in accessing Community Resources (calling, making appointments, etc). She would likely prefer to start with 3-4 hours a week and increase if needed.

DESCRIPTION OF INDIVIDUAL: Individual is 48, verbal, and fully ambulatory. She had a lot of independent skills when she was younger and has been working to regain them. She is able to self-advocate and has strong spoken and reading language skills.
PROVIDER REQUIREMENTS: Able to provide HPC and FTN. Cooking/cleaning skills ideal.
TRANSPORTATION NEEDS: Non modified vehicle.
DIAGNOSES: Moderate ID, MDD, Anxiety
HOUSING: Lives in Madison
DAY PROGRAM/EMPLOYMENT: Attends VH program
