DESCRIPTION OF REQUESTED SERVICES: Individual is a 34-year-old male looking for weekly HPC hours (estimated 8 hours per week). These hours can be after day program or on weekends per the need of the family. Prefer a male staff member. SEEKING AGENCY OR INDPENDENT PROVIDER: Independent or agency provider works but prefer male staffing.

DESCRIPTION OF INDIVIDUAL: Individual will need support to manage his funds in the community & follow pedestrian safety laws. He may need assistance for medication administration depending on the time of the HPC hours. This individual may ask for his back, neck or head to be rubbed as a means to regulate himself. Keeping a consistent routine is important to him. He does best when plans are made in advance, and he has specific details as to what to expect. It is important for the individual to have a quiet environment and space to himself. It is also important for him to have access to his computer/laptop, which he uses to look up topics of interest (trains, airplanes, gas stations, bees, and insects). He loves the Natural History Museum, Cats, going to stores, and going to Penitentiary Glenn. Family believe he would be open to going to the movies, and he also loves Star Trek. He is working on using a communication device as he is not always easily understood.
PROVIDER REQUIREMENTS: HPC & Transportation. A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism, OCD/anxiety, Trichotillomania (pulls hair out), Excoriation (chronic skin picking), ADHD, Mild ID
HOUSING: Individual is currently living with his parents/guardian in the Madison area.
DAY PROGRAM/EMPLOYMENT: Eastwood day program 4 days per week.
