DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC/ATN services approximately 3 days per week; Up to 15-20 hours per week. He is requesting assistance with attending a gym, grocery shopping, attending doctor appointments, meal preparation, and routine cleaning of his home.
SEEKING AGENCY OR INDPENDENT PROVIDER: Individual does not have a preference but requests consistency with scheduling.

DESCRIPTION OF INDIVIDUAL: Individual is independent with all ADLs and Medication Administration. He needs encouragement and assistance to maintain a healthier lifestyle to reduce the risks associated with Fatty Liver Disease. He does not particularly care for participation in group activities. He is a loner by nature and enjoys 1:1 time with others. At times, he needs encouragement to make good decisions. Although he will make his own decisions, he will accept guidance and information to make an educated decision. He has dogs, which can be intimidating, but he will crate them when staff are in his home. The individual loves animals and enjoys watching sports.
PROVIDER REQUIREMENTS: Provider will need to be able to deliver all services identified in the ISP. Certification in Medication Administration and training in crisis intervention is not required by provider staff.
TRANSPORTATION NEEDS: Individual is able to ride in a non-modified vehicle.
DIAGNOSES:
HOUSING: Individual resides in a detached garage that was remodeled into an apartment. He will require some services delivered in his apartment and some services to access the community.
DAY PROGRAM/EMPLOYMENT: Individual is independently employed in the community and currently works 3 days per week (M, W, F). His mother typically provides transportation to/from work, but on occasion, the individual may request assistance from HPC staff, if available.
