DESCRIPTION of INDIVIDUAL: Individual is a 46-year-old male. He enjoys a variety of activities, including music, dancing, bowling, shopping and going movies. He is verbal and very kind. He likes to socialize, stay busy, and spend time with his girlfriend and her family.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: He has a diagnosis of Down syndrome, Depressive Disorder NOS, Obsessive Compulsive Disorder, and Mild Intellectual Disability
HOUSING: Individual currently lives in a congregate setting in Madison.
DAY PROGRAM: Attends day program 5 days a week.
SUPPORTS NEEDED: Individual needs assistance with purchases and visual supervision in the community. He will need transportation to all activities, and assistance to schedule and attend medical appointments, haircuts, preferred activities, etc. Physical assistance to clean and maintain his home, as well as preparing and planning all meals. Verbal reminders for most of his hygiene tasks and needs physical assistance shaving and caring for nails.
