DESCRIPTION of REQUESTED SERVICES: The individual is a 16-year-old residing with in a congregate setting out of county and would like to move closer to his family in Lake County. He receives services in his home to assist him with increased communication skills, light ADL skills, and participation in leisure activities. He does engage in self-injurious and destructive behaviors when upset, and may become aggressive when others intervene, so he would need staff experienced in co-regulation and de-escalation. Historically these episodes are rare, and he has had significantly fewer instances of aggression or destruction in recent months. Elopement continues to be a concern, as he will run from a caregiver given an opportunity. He communicates with an AAC device.

DESCRIPTION of INDIVIDUAL: Individual has limited communication skills and benefits from assistance to relay his wants and needs through the use of icons/pictures, gestures and a communication device. Receptive language skills are very high. He requires a carefully structured environment and implementation of positive behavioral strategies to reduce incidents of physical aggression and property destruction. Individual loves to move, and does well with room to pace/race, climb, and/or swing to keep regulated.
PROVIDER REQUIREMENTS: HPC/ATN, crisis prevention training preferred
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism Spectrum Disorder, Moderate Intellectual Disability, Macrocephaly, Expressive Speech and language development delays
HOUSING: Currently lives in congregate setting
DAY PROGRAM/EMPLOYMENT: Attends school
