DESCRIPTION OF REQUESTED SERVICES: This individual is a 45-year-old female who resides in Concord. She is looking for HPC services Monday and Fridays 2-4 hours a day to help her access the community. Individuals prefer female staff who can drive her places into the community. Individual has not been going out into community recently and needs assistance identifying activities she may enjoy.

DESCRIPTION OF INDIVIDUAL: This individual requires auditory supervision in her home and visual supervision in the community. She is verbal but can be difficult to understand as she speaks quickly. Individual may also make loud noises or yell, but this behavior is not related to any aggressiveness. This individual does require physical assistance with wiping after a bladder or bowel movement. She may also require some reassurance that her family and belongings are at home and safe. These individual loves having an active social life where she can have conversations with others. She loves to hear about pets and babies and enjoys playing with toys.
PROVIDER REQUIREMENTS: This individual prefers female staff
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Moderate ID, Attention Deficit Hyperactivity Disorder, Cyclothymic Disorder
HOUSING: This individual lives at home with her mother, stepfather and cats. HPC services to be provided in the community
DAY PROGRAM/EMPLOYMENT: No day program or employment at this time
