DESCRIPTION OF REQUESTED SERVICES: This is a request for a 21-year-old male seeking Service Animal Training supports. Team is looking for Seizure alert training dog to detect seizure activity of the individual within his home.

DESCRIPTION OF INDIVIDUAL: This individual enjoys spending time watching his shows, listening to music on an ipad, and going for car rides out in the community. He is non-speaking and communicates through body language, gestures, vocalizations, and a speech generative device. He enjoys spending time in the car and going for walks in his adult push chair when available. Individual does have a history of physical aggression towards staff at home.
PROVIDER REQUIREMENTS:
TRANSPORTATION NEEDS: Individual requires a non-modified vehicle to access the community.
DIAGNOSES: Global Developmental delays, ataxia, cerebral palsy, visual impairment, and seizure disorder
HOUSING: Individual resides by himself in a condo purchased by family.
DAY PROGRAM/EMPLOYMENT: Individual currently attends school and will be graduating in May of 2026.
