Join us on Wednesday, April 20, to learn what you need to know as an Authorized Representative of Medicaid Benefits and/or a Representative Payee for Social Security Benefits. This Forum begins at 6:00 p.m. and will provide a practical look at your responsibilities and is intended to be informational, covering topics of interest to the individuals and families served by the Lake County Board of DD/Deepwood.

• In-person attendance will be at the Lake County Board of DD/Deepwood – VGC Building, located at 8121 Deepwood Blvd, in Mentor, OH.
• If you wish to participate as a remote attendee, you will be given a Zoom link when you register.
