Annual Input Survey Open Now through November 26, 2021

The County Board is in process of developing our annual goals for 2022, and we would like to offer you the opportunity to read about what we’ve accomplished in 2021. Since we value feedback from the individuals, families, and guardians of individuals who receive services through the Lake County Board of Developmental Disabilities/Deepwood, we’d like to collect feedback as part of our planning process.

For those who do not have computer access and need a hard copy of the annual goals sent via U.S. mail, we are asking them to email Annual.Input@lakebdd.org or call (440) 350-5032. Please share this information and help us create awareness about the opportunity to provide input so that we can be sure we are planning for changes appropriately while meeting the needs of the individuals we serve in Lake County, Ohio.
