Download and complete the order form, mail it to or drop it off at the Lake County Board of DD/Deepwood c/o Lynnmarie at 8121 Deepwood Blvd. Mentor, Ohio 44060 by April 25. You may pick up your order after May 22 in the building where you receive services or at the Administration Building at 8121 Deepwood Blvd, Mentor, Ohio 44060.
