Our Agency is searching for Service and Support Administrators.

Essential Job Duties (not all inclusive):

  • Establishes an individual’s eligibility for the services of the Lake County Board of DD/Deepwood and referral to appropriate Board or community services and/or waiting lists.
  • Maintains management and accountability of the Lake County Board of DD/Deepwood waiting lists for services.
  • Assesses individual needs for services including, but not limited, to Pre-Admission Screening and Residential Reviews, Level-of-Care (LOC) Determinations, Initial Needs Assessment, etc.
  • Develops one comprehensive Individual Service Plan (ISP) for assigned individuals with the active participation of the individual to be served, other persons selected by the individual, and, when applicable, the provider(s) selected by the individual, and recommends the plans for approval by The Ohio Department of Developmental Disabilities (DODD) when services included in the plans are funded through Medicaid.
  • Coordinates the delivery of various services specific to the individual (IO, SL, SELF Level 1, FRS, Day Services, Transition Service, Employment Navigation, etc.).

Requirements:

  • Bachelor’s Degree in related field
  • Valid State of Ohio driver’s license
  • Availability of personal vehicle for routine and crisis response.

 Hours & Job Location: 

  • Full Time in Mentor, Ohio
